Your eligibility is based on factors to do with your health, how you manage at home and if you are currently receiving support. You could be eligible for services if you have:
– noticed a change in what you can remember or what you can do
– experienced a change in care arrangements
– experienced a recent fall or been admitted to hospital
and are:
– 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people)
To know if you are eligible, you will need to have an assessment done.
